You should see the Forgot Password En-trance if you already have a security email set up. Follow the steps to reset your password. Tap the entry and follow the directions.

You can recover your data from cloud storage if you don't have a security email but had back up to the cloud.

Vault may be deleted by some cleaning and free storage apps. This is because Vault uses the data folder to save videos and pictures. So as a best practice please do not choose to delete Vault's data folder and subfolders (mnt/sdcard/SystemAndroid) when you use such apps.
